JOB DETAILS
Provides the Service Area with nursing field supervision of the In-Home Aides and client care coordination. Responsible for conducting skilled nursing visits and on-site reassessments. Assist with orientation and in-service training of the aides.
Supervisory
Responsible for the field supervision of the In-Home Aides.
Performs quarterly performance appraisals and annual evaluations with recommendation of salary and/or job description changes relevant to the personnel of this area.
Maintains ongoing documentation of employee performance, implementing disciplinary action if necessary.
Other
Responsible for processing intakes for new clients and submitting them within 48 hours. Responsible for processing on-site client.
Conduct skilled nursing visits as regulated by the State of Missouri, Division of Aging or Department of Health. Submit physician's orders every sixty (60) days.
Provide patient education as well as medical consulting to staff.
Responsible for maintaining a Registered Nurses log.
Assist with monitoring when needed. Monitor the appropriateness of client care plan to meet client needs within the mandated regulations. Monitor the conditions in the home which may pose a safety hazard to the client or employee. Monitor the worker's ability to display the skills needed to successfully implement client's care plan.
Investigates client grievances thoroughly with signed client statements and documentation as needed.
Assists in interviewing and hiring of new aides.
Assists with training of the new aides.
Provides Medication Management Services per MARC contract to caseload of clients
Other duties may be assigned by the Supervisor and/or Chief Executive Officer.
