JOB DETAILS
The Archdiocese of Philadelphia is currently seeking a Case Manager to work at the Women of Hope, at our Lombard St. location in Center City Philadelphia.
BA/BS required, BSW preferred. Must have experience/knowledge in working with women who have mental illness and have been homeless. Must have good organizational, communication and computer skills. Duties include: providing on-site case management with residents, assisting residents with activities of daily living, maintaining weekly documentation to include care plans, progress and medical notes, and attending regular staff meetings and required trainings. Case managers also assist in the administration of medications, accompany residents on medical appointments, and some recreational outings. On call responsibilities on a rotation schedule with other staff.
