Job Description

Case Manager (RN)

Location: Merion, PA - Mercy Convent
Schedule: Full-Time (30+ hours/week)
Salary Range: $75,000-105,000 (Based on experience)
License Required: RN - Pennsylvania
Experience Required: 3-5 years

About the Role

The Case Manager supports the overall health and wellness of the Sisters residing in the community. This role works collaboratively with residents and the Health & Wellness team to assess needs, develop care plans, coordinate services, and advocate for resources that promote safety, autonomy, and wellbeing.

Grounded in the mission of the Sisters of Mercy-to serve with compassion, justice, and mercy-this position plays a meaningful role in providing holistic, person-centered care to religious women dedicated to lifelong service.

Key Responsibilities

Care Management & Clinical Support

Conduct Symbria Wellness profile assessments on schedule.
Manage chronic conditions, new diagnoses, and progressive illnesses; create, implement, and update care/service plans.
Participate in discharge planning and transitions of care.
Provide hands-on nursing care including vital signs, assessments, wound care, injections, and first aid.
Coordinate and accompany Sisters to medical appointments; communicate with providers; handle ER transfer accompaniment as appropriate.
Assist with follow-up appointments, scheduling, and transportation arrangements.
Coordination & Advocacy

Serve as a liaison with McAuley Convent and other internal/external service providers.
Recommend and coordinate referrals to rehab, behavioral health, homecare, and specialty services.
Support Sisters in navigating healthcare systems and accessing services.
Provide case management for Sisters living in the broader community.
Deliver education, coaching, and health/wellness counseling in collaboration with the care team and Life Enrichment Coordinator.
Safety & Compliance

Follow all safety protocols and report hazardous conditions immediately.
Maintain clean, safe work environments and identify potential risks.
Demonstrate strong working knowledge of emergency procedures and respond effectively during crisis situations.
Required Qualifications

Active RN license in Pennsylvania (RN/BSN preferred).
3-5 years of experience in geriatric care, long-term care, or homecare.
Experience with case management and admission/transfer/discharge processes.
Comfort working in a healthcare setting serving religious women.
Strong clinical judgment and crisis-management skills.
Strong organizational, communication, and interpersonal skills.
Ability to work independently while collaborating effectively within a team.
Commitment to a holistic, person-centered approach to health and wellness.
Physical Requirements

Ability to lift, push, and pull up to 50 lbs.
Ability to sit or stand for long periods.
Frequent bending, walking, kneeling, reaching, and manual tasks.
Clear vision, hearing, and communication in English.
Ability to read small print and distinguish basic colors.

Pay: $75,000.00 - $105,000.00 per year
