Job Description

TITLE: Case Manager
REPORTS TO: Director of Programming
FLSA CLASSIFICATION: Full time, Exempt

SUMMARY: The SSJ Newcomer Housing Alliance (SSJ NHA), a sponsored ministry of the Sisters of Saint Joseph, provides transitional housing and wraparound services for newly arrived refugee and immigrant individuals and families in Philadelphia. Through our services, we help families transition to stable, independent housing. The Case Manager guides clients through the transition to long-term stability by delivering culturally responsive, strengths-based services. Working closely with the Director of Programming, the Case Manager coordinates individualized plans, connects clients to resources, and tracks progress toward self-sufficiency.

ESSENTIAL FUNCTIONS:
• The Case Manager commits to the Mission/Vision of the Sisters of Saint Joseph.
CASE MANAGEMENT
• Complete intake assessments and orientations to identify needs, set goals, and establish a supportive working relationship.
• Provide direct services including enrollment in public benefits, school registration, job search assistance, and advocacy with partners and systems.
• Develop, implement, and regularly update individualized service plans in partnership with clients.
• Make appropriate referrals to community services (e.g. employment, legal, healthcare, schooling for children, and housing).
• Conduct a minimum of two quality client contacts/visits per case each month.
• Maintain timely and accurate case notes; track outcomes and progress toward goals.
• Facilitate the discharge process by providing training on housing rights, budgeting, and related topics.
• Support families in achieving financial independence (e.g., creating budgets to manage rent, utilities, and household expenses).
COLLABORATION & PROGRAM SUPPORT
• Serve as a liaison among staff, volunteers, and families to ensure coordinated support.
• Support and coach volunteers to strengthen their capacity to assist assigned families.
• Partner with SSJ NHA staff to plan and lead family programming, such as educational workshops, holiday events, and recreational outings.
• Coordinate with external partners and service systems to remove barriers to housing stability and integration.

• OTHER
• Advocate with landlords, schools, and government agencies as needed.
• Stay current on relevant guidelines, regulations, and changes in immigration policy..
• Perform tasks to ensure the success and alignment with the mission of the SSJs.
• Complete other responsibilities as needed for the SSJ Newcomer Housing Alliance ministry.
EXPERIENCE:
• Minimum of 2 years working with refugee, asylum seekers, or other immigrant populations
• Strong understanding of immigration-related challenges and available community resources.
• Experience working within social work, human services, or community-based organizations.
• In-depth experience working in the field of social work/human services
• Proven track record of successfully managing multiple demands on a day-to-day basis
EDUCATION: Bachelor's Degree in social work or related area required; Master's Degree preferred.
QUALIFICATIONS:
• Bilingual in Spanish or French preferred.
• Valid driver's license and access to a personal vehicle for home visits across the Philadelphia regio
• Strong organizational, documentation, and time-management skills.
• Excellent verbal and written communication.
• High level of cultural humility and cross-cultural sensitivity.
• Creative problem-solving and adaptability in working with families with complex needs.
• Ability to maintain confidentiality and professional boundaries.
