Job Description

Job Title: Program Manager

Type of position: Full-time position

Position Description:
Reports to the Executive Director. The Program Manager works directly with residents to help them meet their goals and track their progress.

Responsibilities:

Mom Responsibilities:
In conjunction with Annie's Home staff, creates self-identified measurable goals to ensure families are successful in accomplishing their identified goals.

Provide moms with the resources they need to reach personal counseling goals.

Coordinate and create programs for current moms that reflect the mission, vision and core values of Annie's Home.

Occasionally participate in community night rotation. Coordinate Monday night room checks.

Coordinate the evening program for the current moms (examples include infant safety, life skills program, baby milestone program, etc.)

In conjunction with the Executive Director, coordinate drug testing of residents.

Monitor and track client progress and modify their treatment plans as needed.

Completes case notes, move in and move out paperwork, progress notes, assessments, and other chart-related documents and correspondence for assigned clients in a timely manner and according to organizational policy.

Run weekly staff meeting to help monitor progress of all current residents.

Seeks consultation with staff team members as necessary.
Provides referrals for individuals who need more specialized care, working with other community resources and referral sources to coordinate services for such clients.

Provides therapeutic crisis intervention and emergency services as needed and/or available.

Adheres to confidentiality law and procedures at all times.
Obtain signed release of information authorization form from client.

Facilitates Seeking Safety curriculum for current moms on an individual and group basis.

Intake Responsibilities:
Helps service initial phone inquiries and conducts phone screenings for individuals applying for services to assess if Annie's Home is the appropriate housing program for the individual.

Coordinates in-person interview assessment that accommodates applicant's schedule and current location, gathers pertinent information and determines eligibility during the interview process.

Refers applicants to appropriate housing agencies if determined ineligible for Annie's Home and keeps record of these referrals.

Conducts consistent and timely follow-up with active applicants.

Collaborative Efforts:
Serves in the on-call rotation for in-home emergencies.

Participate in weekly meetings with appropriate teams for easy and transparent information sharing.

Consult the leadership team at home prior to asking a mom to leave.

Educate staff and clients on stress and trauma coping mechanisms.

Be available for emergency/crisis/major issues that arise at the house. Provide onsite support to the staff during crisis or emergency situations. Be intentionally aware of conflicts, difficulties or stressors. Coordinate with a mental health provider to deescalate the situation. Follow up with the Executive Director.

Mediates and leads important conversation/safe space conversations with moms, staff, volunteers etc as needed.

Assumes any additional projects or tasks as assigned by the Executive Director.

Preferred Qualifications:
Bachelor's degree in social work, counseling, or other related therapy
Independently licensed LSW or LISW in the state of Pennsylvania

Knowledge of resources in the Philadelphia community
Excellence in organizational, supervisory and communications skills.

Experience working with vulnerable populations, addiction and poverty.
