Summary of ResponsibilitiesThe Case Manager will be responsible for case management. The CM's primary purpose is to assist residents to move toward achieving the goal of family, self-development, independence by providing case management, some individual coaching, and supportive services, including the maintenance of safe, secure and healthy housing for expectant and parenting single mothers and children. The CM assists in developing, monitoring, and evaluating treatment plans and progress. The CM will provide resident care by assessing treatment needs. CM will coordinate and assist with services for residents by linking residents to and advocating for appropriate and organization accepted social services and community resources. CM is to monitor and evaluate treatment plans and progress. CM is responsible for new resident's initial interview, intake, admission, and on boarding. CM will document all contacts with and progress of residents. CM is responsible for producing required and requested reports by deadlines.
The CM interacts directly with the House Mom/House Manager in planning for each resident individually to bring healing and wholeness to her.
The CM is to maintain confidentiality to protect residents' privacy. The CM position is a part-time position requiring a minimum of 20 to 30 hours per week.
Responsibilities include but are not limited to:
• Conduct new resident's initial interview, intake, admission, and on boarding.
• Conduct daily property walk-thru of house to ensure the residents are following and doing the program.
• Maintain all employment documents for the residents.
• Organize regular house meetings and trainings for the residents; oversee resident's responsibilities; communicate with POS, House Mom and Mentor on resident needs/issues, policy changes, and other important information.
• Embrace and uphold the mission and values of Mary's Choice RVA home.
• Assess new resident's physical and mental health, legal history, education and learning abilities, work history, and substance abuse history.
• Meet with residents on a regular basis, no less than once per week, to provide guidance, advisement, and assessment toward resident goals. The frequency of meetings may be determined by the level of care needed on a case-by-case basis.
• Coordinate and manage residents' appropriate educational, vocational, medical, social and other services as needed.
• Work with Program and Operations Specialist, Resident Relations Specialist, House Mom/Manager, Relief House Mom/Manager and Mentors to ensure residents are meeting program and personal goals; establish corrective action if needed.
• Coordinate and monitor resident participation in outside counseling services, recovery programs (AA, NA, Celebrate Recovery, smoking cessation, etc.), parenting skills development, social service resources, vocational training, school or GED program.
• Provide ongoing resident emotional support, including crisis management and intervention.
• Identify and assess community resources to address specific needs of individual residents.
• Serve as a weekend on-call program staff person and lead weeknight resident meeting on a rotating basis with the Program and Operations Specialist, and Resident Relations Specialist.
• Provide transport, accompaniment, and support of residents to medical services, court appearances, and social services agencies as needed.
• Work with Commonwealth Catholic Charities Social Worker regarding residents counseling program needs.
• On-call for transport of laboring residents; serve as labor coach if requested by resident.
• Work with Commonwealth Catholic Charities Social Worker in connecting residents with community services, serve as a liaison to CCC Case Manager
• Enter clear, accurate, and thorough documentation of 1) all inquiry information, 2) intake information and outcomes, 3) all contacts with residents, 4) reports, 5) applicable treatment plans, 6) resident outcomes 7) other data as necessary.
• Demonstrate respect, compassion, and cultural sensitivity to all residents, inquiries, and referrals.
• Report and document significant information regarding any unusual incidents or accidents occurring with residents and/or within area of responsibility.
• Network with community resource providers for referring non-eligible inquiries.
• Outreach to referral sources such as pregnancy resource centers, school counselors, medical professionals, social service agencies, shelters, etc. for potential residents.
• Provide residents with appropriate guidance, advice and training in building skills such as parenting techniques, appropriate discipline, communication, problem solving; conflict resolution, self-sufficiency, personal responsibility, responsible thinking, etc. in individual and/or group settings.
