POSITION SUMMARY
Reporting to the Director of Administration, the Senior Coordinator is responsible for ensuring the health and wellness programs, and services of the region effectively support the quality of life for each sister in all stages of life as an individual and in community. The Senior Coordinator is responsible for successful implementation of the Health and Wellness plan of the Region, which will include travel to locations across the US and Toronto, Canada.
The Senior Coordinator will work in close collaboration with the Regional Directors and Leadership Team. The Senior Coordinator will provide leadership direction and oversight for partners responsible for health and wellness services, including employees reporting to them, employees who report to a local community supervisor, sisters caring for other members, and third-party service providers.
The Senior Coordinator acts on a high level, however they will occasionally be expected to provide direct care to sisters, be available to respond to emergencies, or be with a sister for certain treatments, surgeries or other health-related events.
RESPONSIBILITIES
Health and wellness plan and programs
• In coordination with Leadership will establish, implement, communicate, and maintain the Region's health and wellness plan and programs. This includes responses to health crises such as pandemics or other emergencies.
• Supervises Health and Wellness employees to ensure compliance and performance.
• Establishes and implements processes and systems to continually assess and meet the health and wellness needs of the sisters.
• Reports regularly to the Directors and Leadership Team on the status of and projected needs for health and wellness across the region.
• Helps develop and identify existing education programs regarding health and wellness for individual sisters, communities, and Region staff.
• Collaborates with other members of the Team to further develop the care of the sisters and to provide a standard of care consistent with the values of the Sisters of Good Shepherd and the Health and Wellness Plan.
• Will continue professional development to remain awareness of best practices in the areas of women's health, eldercare, gerontology, and health and wellness for the aged.
• Assists in selection and monitors performance of health and wellness service providers, including housing and medical care. Makes recommendations for changes in providers when services do not meet standards.
• Work directly with staff at third-party care facilities that the sisters live in and will also be in contact with any third-party outside resource that provides care services.
Individual sister care
• Ensures every sister creates and maintains an up-to-date individual Wellness Plan addressing stage of life, health-related needs, risk factors as well as services to address these areas in ways that are consistent with Region and community goals and values. Periodically reviews each sister's wellness plan.
• Assists and advises individual sisters needing assistance with healthcare decisions, finds resources and sources referrals, and related services.
• Ensures every sister has regular contact virtually, by phone or in-person, according to her needs, with focus on those sisters identified as in need of particular care and assistance.
• Provides or ensures the provision of care management services to sisters based on their Wellness Plans. Examples of services include providing information and referral to health and wellness professionals, accessing home and community-based services, accessing higher levels of care and providing ongoing support.
• Offers support and guidance to sisters in modifying lifestyles, adjusting to disease, coping with chronic illness, and other health concerns and supporting sisters in times of crisis.
• Refers sisters to the proper person in leadership when required.
• Assists with visiting hospitalized sisters and helps in planning for discharge.
• Approves and may arrange special orders regarding equipment, medications, and medical supplies.
• Identifies and provides recommendations to the sister and to Leadership when the need for a higher level of care is required.
• Collaborates with other care providers as part of a care team and helps in finding resources who could provide an additional level of backup expertise and assistance.
• Assesses needs and advocates for sisters during acute care episodes, or when specifically requested to do so.
• Assesses and reviews third party facilities or services when new relationships are needed for a sister's care.
• Supervises sister transitions in care levels, facilities or moves whether in the same facility, city, state or across the country.
Documentation and Records
• Ensures the Region maintains confidential up to date health and wellness files on each sister in accordance with policies and, where applicable, HIPPA. Records currently include their Advanced Directives and Durable Power of Health Care Attorney, and end of life directives.
• Assists sisters and staff to update records when changes occur.
• Works closely with Benefits Coordinator to ensure all documentation and records keeping in relation to insurance and other benefits is maintained appropriately.
Performs other duties as assigned.
PHYSICAL POSITION REQUIREMENTS
• Can push, pull or lift and/or carry up to fifteen (15) pounds in order to assist sisters with transportation (getting in and out of vehicles, pushing a wheelchair).
• Manual dexterity as required for health assessments.
• Vision sufficient to read reports and use computers, with or without reasonable accommodation.
• Ability to hear normal conversation, routine telephone communication with or without reasonable accommodation.
• Ability to read, comprehend, reason, interpret and analyze information.
• Competency with written communication.
KNOWLEDGE, SKILLS AND ABILITIES
• Training and direct experience in healthcare, especially in serving persons with chronic health conditions and the elderly;
• Demonstrated knowledge of:
o Health care systems that include acute care, home and community-based services and long-term care;
o Person-directed and holistic models of care;
o Principles of advocacy;
• Strong research skills to identify resources across the Region;
• Able to develop and maintain collaborative relationships within communities and among a variety of healthcare professionals;
• Professional and compassionate communication style;
• Capacity to work effectively with clients who may not request or recognize the need for intervention;
• Proficient with office technologies, use of the internet, Microsoft Office Suite, and other professional computer programs.
WORK ENVIRONMENT
• Regular travel (including overnight stays) is an essential component of the job.
• Remote work when not at a community or sister site; worksites include offices, skilled care facilities, hospitals, medical offices, and residential settings.
• Requires regular use of a cell phone and computer, including for zoom meetings. Will use Microsoft Office Suite, and Outlook email.
• Regularly involves driving and assisting sisters to enter and exit vehicles.
• Schedule requires flexibility, including on-call requirements and occasional evening and weekend hours to be responsive to the needs of the sisters.
• Risks associated with routine office environment and eldercare facility environment.
• Potential and/or direct exposure to blood, body fluids, infectious waste, diseases, conditions (e.g., TB, Hepatitis B viruses).
EDUCATION AND EXPERIENCE
• Masters in social work or related field, or a BSN preferred.
• Minimum 5 years' experience in related field, coupled with license (i.e., LVN/LPN) or certifications related to health and wellness, in lieu of degree.
• Minimum of 3-5 years of supervisory and care management experience.
• Experience working in health care or senior services is preferred.
• Prior experience in senior living, long term care, or assisted living environment is a plus.
• Experience working in a women's religious congregation is a plus.
• Experience and understanding of the Catholic faith and Catholic social teaching is a plus.
CERTIFICATES, LICENSES, AND REGISTRATIONS
• Must have valid and current driver's license, and consent to Department of Motor Vehicle review of past driving record to establish that s/he is a safe driver.
• Preference is a licensed professional in a health-related field.
Work location is open to discussion, as the Senior Coordinator will be supporting sisters and leading staff across the US and Toronto. We would prefer proximity to Cincinnati, New York or St. Louis, where we have offices and/or concentrations of sisters.
The salary range starts at $90,000 and will be commensurate with experience and adjusted for regional differences in cost of living. We offer a comprehensive benefits package that includes health, dental, vision, generous time off, and 403(b).
